The following is information about how to apply for a particular program and the things you should bring with you.

Applying for Health Insurance assistance

If you are:
Under 65 years old you should apply for Medicaid, Child Health Plus or private health insurance through the New York State of Health Marketplace. Eligible individuals can also get help paying for private health insurance through new tax credits to lower costs through the NY State of Health. For private health insurance, the first open enrollment period ended March 31, 2014. The next open enrollment period for individuals to purchase private insurance through the New York State of Health Marketplace is November 15, 2014 to January 15, 2015. Only people with qualifying events can buy private insurance outside of this time period. Medicaid and Child Health Plus are available year round.
If you are:
Over 65 years old or older and would like to apply for Medicaid visit the NYC Human Resources Administration.
If you are:
Seeking long-term care, visit the NYC Human Resources Administration to apply for Medicaid. See below for a link to the description of long term care.
Notes: It is important that everyone who can get insurance does so – that Is because the Affordable Care Act requires it. It requires uninsured individuals to have health insurance starting January 1, 2014, or pay a fee when you file your taxes - unless you can claim an exemption.
