The following is information about how to apply for a particular program and the things you should bring with you.

Applying for Commodity Supplemental Food Program (CSFP)

Here are the things you need to do:
1. To apply for CSFP, please call the distribution site nearest you for application information. In ACCESS NYC, you can search for CSFP locations. You can also get information about CSFP locations, by calling the Growing Up Healthy Hotline at 1-800-522-5006 or through TTY access at 1-800-655-1789.
2. You will need to bring documents such as proof of identity, residency and income. In ACCESS NYC, you can search for the full list of required documents.
3. If you are eligible for the program, you will receive a CSFP card. You will need to go to a distribution site to pick-up your food.
4. You can get CSFP even if you are receiving Supplemental Nutrition Assistance Program (SNAP) Benefits.
