The following is information about how to apply for a particular program and the things you should bring with you.

Applying for Nurse-Family Partnership (for first time pregnant women)

Here are the things you need to do:
1. Nurse-Family Partnership is a program for first time pregnant women who are less than 28 weeks pregnant.
2. The Nurse-Family Partnership referral form should be completed. You can view and print this form from ACCESS NYC.
3. To enroll in the program, you must contact one of the Nurse-Family Partnership locations. Call 311 or contact one of the Nurse-Family Partnership locations directly. In ACCESS NYC, you can search for Nurse-Family Partnership sites.
4. There are no supporting documents required to apply for Nurse-Family Partnership.
